Eosinophilic cystitis

Rare inflammatory bladder condition involving transmural inflammation with predominance of eosinophils.

Unclear aetiology – postulated to result from immunological response to an antigen.

Can occur in adults and children.

 

Typically presents with frequency, dysuria, haematuria, and pain.

 

Cystoscopy generally shows erythematous, polypoid, velvety red lesions and gross mucosal oedema.

Histologically there is inflammation with eosinophils, most intense in lamina propria, and occasionally associated with muscle necrosis.

 

Management is generally conservative.

NSAIDs and anti-histamines have been used.

Corticosteroids have been used.

Other reported treatments include azathioprine, cyclosporin, intravesical DMSO and diversion.
