Asymptomatic bacteriuria

  • EAU definition “without urinary tract symptoms…mid stream sample showing > 105 cfu / mL in 2 consecutive samples in women, or one single sample in men”
    • As low as 102 in catheterised samples

  • Screening and treatment for bacteriuria is not recommended
  • 15 – 50 % incidence in nursing home patients
  • Patients with long term catheters, those who self catheterise, or those with reconstructed urological tracts will often be colonised with asymptomatic bacteriuria, with no proven benefit in treating

  • Worth screening and treating in:
    • Pregnancy – 4 – 7 % incidence (similar to non pregnant women) but 20 – 40 % progress to pyelonephritis if untreated
    • Prior to urological surgery
